Advance Health Care Directives

Every person who can understand the issues can make decisions about his/her health care treatment choices and organ donation. This can be done by completing an Advance Medical Directive and/or a Durable Power of Attorney for Health Care.

Decisions

  • An Advance Medical Directive lets your health care provider know if you do or do not want to be kept alive if you are in a coma that you will not wake up from, or if you have a sickness that you will die from. This used to be called a “Living Will”.
  • The Durable Power of Attorney for Health Care Decisions lets you name another person to make medical decisions for you if you are no longer able to make those decisions yourself.
